Job summary

Enriched Living is seeking a Community Support Professional to manage a small caseload and provide community-based support. The role includes transportation to appointments, direct client support, and navigation of community resources.

Flexibility with a 9:00 AM – 7:00 PM schedule, including occasional evenings and weekends. The position offers full-time and part-time options with benefits such as vision, health, HSA, PTO, and FSA.

Qualifications

• Bachelor’s degree or higher in a human services field with relevant direct care experience.
• Associate or Diploma with required years of experience.
• Willing to complete background checks and vehicle-operations requirements.

Responsibilities

• Manage a caseload of 2–5 individuals.
• Attend and provide transportation to appointments and community engagements.
• Provide direct, person-centered support in community settings.
• Assist individuals in accessing and navigating community resources.
• Maintain accurate, timely documentation using a mobile device.
• Ensure compliance with 245D licensing and service standards.
• Collaborate with support teams to promote positive outcomes.
• Advocate for individuals and represent Enriched Living with professionalism.
