Wellvance is seeking a Community Living Support Professional to help individuals build skills and independence in their communities. You will work with clients based on Person-Centered Plans, teaching daily living activities and supporting their community participation.

Role requires travel within the service area, strong communication, and the ability to manage documentation while adhering to agency guidelines.

Qualifications

• Ability to work flexible schedules and travel within our service area.
• Valid Michigan driver’s license, vehicle insurance, and good driving record.
• U.S. citizenship or legal authorization to work in the U.S.
• Lived experience with mental illness, developmental disabilities, or substance use disorders is valued.

Responsibilities

• Support individuals based on their Person-Centered Plan.
• Teach daily living skills like meal prep, budgeting, and personal care.
• Assist with community inclusion and social activities.
• Advocate for individuals and promote recovery principles.
• Maintain accurate documentation and follow agency guidelines.
• Work flexible hours, including evenings, weekends, and holidays.
