Job summary

Enriched Living seeks a dedicated Community Support Professional to manage a small caseload and deliver community-based supports in Saint Paul, MN. The role includes transportation to appointments, direct, person-centered assistance, and collaboration with support teams to promote positive outcomes.

Ideal candidates will have experience with diverse mental health diagnoses and a valid driver’s license, with scheduling Monday–Friday, 9:00 am–5:00 pm and occasional local events.

Qualifications

• Bachelor's degree in a human services-related field is required with 1 year of direct care experience.
• Associate degree in a human services-related field with 2 years of direct care experience.
• Diploma in a human services-related field with 3 years of direct care experience.
• Alternatively 50 hours of relevant training and 4 years direct care experience under supervision.

Responsibilities

• Manage a caseload of 2–5 individuals.
• Attend and provide transportation to appointments and community engagements.
• Provide direct, person-centered support in community settings.
• Help individuals access and navigate community resources.
• Maintain accurate documentation using a mobile device; ensure 245D licensing compliance.
