Qualifications

• Bachelor's Degree, or; Any four-year combination of higher education and qualifying experience, or; Four years of qualifying experience.

Responsibilities

• Provide community support services to children and adults in the CPRC program.
• Maintain a caseload of 20–25 clients and meet with them weekly.
• Document client interactions per agency guidelines and deadlines.
• Provide training/education to clients in daily living skills.
• Monitor treatment plan goals and maintain contact with the treatment team.
• Refer clients to appropriate resources to improve functionality.
• Document according to policies and program standards.
