Community Health Resources (CHR) seeks a Community Support Worker to provide direct care and coordination for families in Willimantic, CT. The role involves home and community-based engagement, collaboration with DCF and local services, and a focus on developing care plans based on family needs.

The position emphasizes autonomy within a team, with regular supervision, schedule flexibility, mileage reimbursement, and a company vehicle for shared use. Strong interpersonal skills are essential.

Formación

• Bachelor’s degree in Human Services preferred; associate’s degree plus two years’ experience in related field in lieu of bachelor’s, or high school diploma with minimum 4 years’ related experience will be considered.
• Minimum of 2 years’ experience in mental health related setting.
• Valid driver’s license.
• Bilingual in Spanish and English preferred.

Responsabilidades

• Assesses whole family needs including basic needs, therapeutic, medical, and other needs.
• Provides support and crisis management related to family needs.
• Assists in linking families with needed community support services, programs, and agencies; advocates for clients’ rights and access to services.
• Works as liaison with DCF, hospitals, other community-based services, family members, and significant others.
• Processes appropriate requests for flex funding.
• Works in collaboration with Parent Navigators.
• Additional duties as assigned.
