Job summary

The Community Support Worker role for the Community Support for Families program in Willimantic, CT is a full-time on-site position (40 hours/week) with a Monday–Friday schedule and occasional evening shifts. The role emphasizes autonomy within a collaborative team, community-based visits, and driving with mileage reimbursement and a company vehicle.

Qualifications

• Bachelor’s degree in a Human Services related field preferred; associate’s degree plus two years’ experience in lieu of bachelor’s; or high school diploma with 4 years’ related experience will be considered.
• Minimum of 2 years' experience in mental health-related setting.
• Valid driver’s license required.
• Bilingual in Spanish and English preferred.

Responsibilities

• Assesses whole family needs including basic needs, therapeutic, medical, and other needs.
• Provides support and crisis management related to family needs.
• Links families with needed community support services, programs, and agencies; advocates for clients’ rights and access to services.
• Works as liaison with DCF, hospitals, other community-based services, family members, and significant others.
• Processes appropriate requests for flex funding.
• Collaborates with Parent Navigators.
• Additional duties as assigned.
