Job summary

Springbrook NY, Inc. is seeking a Direct Support Professional to assist individuals across educational, residential and community environments. You will promote independence, support daily living activities, and follow each person’s plan while documenting progress.

Responsibilities include safe personal care, transportation when needed, and helping people pursue meaningful activities. A high school diploma or GED is required; an associate’s degree is preferred.

Qualifications

• High school diploma or GED required; Associates degree preferred.
• Experience working with people with developmental disabilities is preferred.
• Must be able to lift 50 lbs. independently and assist with heavier lifting as needed.
• Duties require professional verbal and written communication skills.
• Proficiency with Excel, Outlook and Word; ability to learn other software as needed.

Responsibilities

• Be a good role model for the people you support in all situations and settings.
• Be an advocate for the people you support in all situations and settings.
• Be punctual and dependable; Regular staff attendance is vital for the people we support.
• In partnership with the people you support, observe, discuss, and help assess their strengths and opportunities to help develop their personal goals and plans.
• Support and encourage people to find meaningful activities and spend time with friends, including community events and recreational activities.
• Follow all written plans, learning materials, and goals.
• Collect required data and complete billing documentation for supports and services provided.
• Obtain and maintain all applicable training, certifications, and knowledge of policies and procedures.
• Ensure safe and clean environments, including grounds and vehicles.
• Attend team meetings and 1:1 meetings with supervisor.
• All other duties as assigned.
• Specific tasks ranging from intensive personal care needs to supervising tasks will depend on each person’s level of need.
