Job summary

Community Solutions is seeking a Community Support Specialist to assist clients and families in Santa Clara and San Benito counties under the Crisis Residential program. The role involves screening, intake, assessments, case management, crisis intervention, and coordination with community providers.

The ideal candidate has a high school diploma or GED with 4 years of direct service experience; a Bachelor's degree or CADC is preferred, and bilingual abilities are a plus.

Qualifications

• Empowerment, advocacy and support techniques for victims/survivors of violence, people with mental illness or substance abuse issues required.

Responsibilities

• Screening, intake and assessments to determine needs and resources; develop TCP and document progress.
• Provide rehabilitation counseling, advocacy and crisis intervention.
• Complete program documents and maintain client charts.
• Coordinate services with community providers and the legal system.
• Teach life skills and assist with educational and support activities.
• Maintain excellent customer service with internal and external customers.
• Develop relationships with community organizations and system providers.
• Must have automobile to perform routine duties.
• Respond to calls and emails within 24 hours and resolve issues using strength-based solutions.
• Participate in meetings and quality improvement activities.
