Job summary

Empowering People's Independence (EPI) in Rochester, NY is seeking a program coordinator to support the Com Hab supervisor in daily operations, staffing, and compliance with OPWDD regulations.

The role requires a high school diploma or GED with 1 year in human services (associates/bachelor preferred), a NYS driver's license, strong communication, confidentiality, and the ability to train staff in SCIP-R, CPR and First Aid if required. Travel is required.

Qualifications

• High School Diploma or GED with 1 year in human services.
• Associates or Bachelors preferred with 1 year in human services.
• Valid NYS Driver's License to operate agency vehicle.
• Maintain confidentiality and HIPAA compliance.
• Ability to train in SCIP-R, CPR and First Aid as required.

Responsibilities

• Assist supervisor with day-to-day operation of the Com Hab program.
• Maintain health and wellness of individuals served per safeguards and plans.
• Attend Life Plan meetings and complete related forms.
• Attend Intake Committee meetings and home visits as part of intake.
• Add new referral information to the electronic record system.
• Ensure all billing components are completed prior to start of service.
• Monitor documentation per OPWDD regulations.
• Provide support to Direct Support Professionals and SAPs.
• Ensure shifts have proper coverage and complete required reports.
• Maintain confidentiality and HIPAA, and participate in staff meetings.
