Job summary

Jewel Human Services seeks a Direct Support Professional to assist individuals with intellectual and developmental disabilities in Day Habilitation, Community Habilitation, Respite, or School Holiday Respite programs.

Under supervision, you will help identify goals, provide support, and ensure respectful, person-centered care. Expect community outings and collaboration with an IDT to implement life plans and maintain records.

Qualifications

  • High School diploma or equivalency preferred.
  • Six months of experience with individuals with intellectual or developmental disabilities preferred.
  • New York State driver's license with a clean driving record required/preferred.
  • SCIP-R, CPR and First Aid certifications preferred.

Responsibilities

  • Support individuals with day-to-day living and community participation.
  • Follow person-centered plans to achieve goals and habilitation objectives.
  • Attend training and maintain confidentiality of records.

Job Summary

Direct Support Professional (DSP) provides various services to individuals with intellectual and developmental disabilities. Direct Support Professionals work in Day Habilitation, Community Habilitation, Respite, or School Holiday Respite programs. Working under the direction of a Program Supervisor, Direct Support Professionals provide services that support the lives of the people we serve.

Essential Duties & Responsibilities
  • Through using Person-Centered Strategies, assist in identifying for each person who you are supporting their likes, dislikes, dream, short-term, and long-term goals.
  • Provide support for each person receiving services.
  • Listen and respect the rights of each person who you are serving and supporting.
  • Arrive and leave work on a timely basis to and from the home, programs site, and any other activity site.
  • Must be able to work your assigned work hours, ready to go outside in the community as the person you are supporting chooses (i.e., volunteer, recreational and place of interest) while working on skill established in the person's habilitation plan, etc.).
  • Build a support network between the individuals and community members.
  • Ensures consistent service delivery that connects work, home, and recreational needs.
  • Must serve as an active member of each person's Inter-Disciplinary Team (IDT), including individual life plan involvement.
  • Attend staff meeting and in-service training as assigned by your supervisor.
  • Respond professionally while in the community and to family.
  • Participate in the orientation seminar and subsequent in-service training. Completing orientation training within the first eight weeks of employment is required to end the probationary period successfully.
  • Maintain service recipient's records and confidentiality of such records.
  • Assist service recipients in attaining community resources, which are appropriate and applicable to their needs and goals.
  • Act as a role model for each service recipient by displaying appropriate behavior, language and offering assistance, as indicated in the life plans.
  • Perform tasks as delegated by your supervisor.
  • Any change in working a shift or working hours must be approved in advance by your supervisor.
  • Responsible for the delivery of value outcomes and training to develop further individual's daily living skills including, but not limited to: budgeting, nutritional planning food preparation, use of public transportation, use of community resources, shopping, money management, housekeeping and domestic tasks, communication and telephone use, health, and self-care, etc.
  • Report all incidents of abuse/neglect per OPWDD and agency regulations.
  • Follow all policies and procedures and other duties as required and requested by your supervisors. When a question arises, seek clarification from your supervisor.
YOU ARE RESPONSIBLE TO
  • Attend required training on Corporate Compliance, documentation, and other topics that relate to your job responsibilities.
  • Read, understand, and sign off on the Jewel Human Services Code of Conduct.
  • Be sensitive to any situation that could lead you or others to engage in actions that violate said policies.
  • Do not represent yourself as having degrees or credentials that you do not have.
  • Ensure that all documentation you complete is accurate, whether related to services you have provided for consumers, financial or personal matters, or other documentation related to your job.
  • If your position requires you to review other people's documentation, do not sign off on it if you do not believe it is accurate.
  • Promptly report any issues, concerns, violations, or suspected violations to your supervisor, Manager of Human Resources, Compliance Officer, or the Director of Operations.

Failure to comply with these policies or to report information regarding suspected improper conduct will result in disciplinary action, up to and including termination of employment, as appropriate.

Qualifications
  • High School diploma or equivalency diploma preferred.
  • Six (6) months of experience working with individuals with intellectual and developmental disabilities or CNA certified preferred.
  • Must have a valid New York State driver's license with a clean driving record.
  • Physically able to bend, stretch, and travel on public transportation and transfer individuals as needed from the bed, chair, and toilet using an approved transfer method.
  • SCIP-R trained, CPR, and 1st Aid certified preferred.
