Job Summary

The Director of Community Services will be responsible for community support activities in accordance with state regulations. This includes overseeing service delivery through assessments, developing care plans, supervising Community Support Specialists, and reviewing documentation for quality and compliance.

Responsibilities
  1. Participate in assessing participant needs and developing, implementing, and monitoring IRP and/or IPP.
  2. Supervise PRP, TCM, and SE services per regulations.
  3. Coordinate care meetings and address client needs.
  4. Communicate with clinical staff regarding progress.
  5. Follow directives from the agency administrator and comply with policies.
  6. Manage fiscal planning and financial responsibilities.
  7. Maintain accurate case records per HIPAA and Medicaid regulations.
  8. Oversee activities ensuring safety, including children’s activities.
  9. Provide supervision, consultation, and crisis intervention.
  10. Conduct trainings and participate in team staffing.
  11. Handle case intakes, authorizations, and verifications.
  12. Act as liaison with other departments and communicate regularly with mental health supervisors.
  13. Collect and review client outcomes, set improvement goals, and report progress.
  14. Attend relevant training sessions.
  15. Perform other duties as assigned.
Qualifications
  • Licensed Mental Health Professional (LGPC, LGSW, LCPC, LCSW-C, LCAD) with 2+ years of relevant experience.
  • Deep knowledge of PRP, TCM, SE, and Medicaid procedures.
  • Knowledge of resources for individuals with serious emotional disturbances.
  • Ability to multitask and work independently in a non-traditional environment.
  • Proficiency in Microsoft Office and electronic systems.
  • CPR/First Aid certification (must be maintained).
  • Bilingual skills are a plus.
