Job summary

NLACRC in the San Fernando Valley seeks a Consumer Services Coordinator to provide information, advocacy, and service arrangements for individuals with developmental disabilities. You will develop plans, coordinate services, and document compliance while collaborating with an interdisciplinary team.

Role requires ability to meet in homes or community settings, travel as needed, and maintain strong communication. Bilingual Spanish may be needed for some positions.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field; at least two years of related experience.
• A Master’s degree may substitute for experience; bilingual Spanish may be required depending on position.
• Strong understanding of family systems and person-centered planning; independent and team work abilities.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop and monitor program plans for individuals; revise as needed.
• Identify and coordinate services for individuals and their families.
• Document forms and reports per regulations and NLACRC policies.
• Attend interdisciplinary meetings with individuals served, families, and providers.
• Schedule and manage case-coverage and on-call emergency services when required.
