Job summary

NLACRC in the San Fernando Valley seeks a Service Coordinator to provide information, advocacy, and service arrangements for consumers and families. You will participate in developing individualized plans, monitor progress, and coordinate services with families and providers.

Role requires understanding of family systems, person-centered planning, and ability to work independently within an interdisciplinary team. Bilingual Spanish may be required depending on position.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field with at least 2 years' related experience.
• Master’s degree may substitute for experience.

Responsibilities

• Provide information, advocacy, and service arrangements for consumers and families.
• Develop and monitor program plans for individuals.
• Identify and coordinate services for individuals and families.
• Document forms and reports per regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Attend interdisciplinary meetings and coordinate with service providers.
