Qualifications

• Experience in social services, case management, or related field preferred.

Responsibilities

• Connect residents with community resources and supportive services.
• Build relationships with local agencies, healthcare providers, and community organizations.
• Coordinate resident programs, educational workshops, and community events.
• Assist residents in accessing benefits, transportation, healthcare, financial assistance.
• Support resident independence while helping create a welcoming, engaged community.
• Maintain service records, referrals, and program documentation.
