Job summary

Children's Institute seeks a Community School Coordinator to lead school teams in designing and overseeing comprehensive community school programs. You will collaborate with school leaders to implement preventions, interventions, and enrichment activities aligned with the annual plan.

You will coordinate with the FET/clinical teams on referrals, manage partnerships, monitor grants, and ensure program outcomes are tracked and reported.

Qualifications

• Bachelor’s degree in education, social work, public health, or related field.
• Master’s degree preferred.
• Proficiency with data systems and reporting tools.

Responsibilities

• Leads a multi-disciplinary, multi-agency team to assess culture and climate and creates a yearly implementation plan.
• Ensures compliance with billing requirements and tracks program evaluation data.
• Facilitates evaluation and gap analyses and needs assessments onsite.
• Oversees linkage of students needing support and timely interventions and monitors progress.
• Ensures high-quality functioning of health, dental, mental health, and dual-generational services.
• Collaborates with clinical teams to triage mental health referrals.
• Supports attendance initiatives and multi-tiered interventions.
• Builds relationships with admin, staff, partners, and community to address needs.
