Job Description Summary:The Community Resources Coordinator/Priority Populations Coordinator manages supportive services, resource coordination, education and benefits navigation for priority populations within the community. Specifically, this position will provide information and referral services to individuals under the age of 60 who are living with a disability. In addition, this position is responsible for implementing the agency’s National Family Caregiver Support Program including education/outreach, screening, referral and coordination of services as well as documentation and reporting. This role works within the Wellness Team at the Office on Aging. Harford County Government Core Values are essential to successful job performance and this position, as with all positions at Harford County, is accountable for integrating Core Values in the execution of responsibilities.Job Description:Budgeted Salary: $54,080 annuallyEssential Duties:Program Coordination: Oversee daily operations, participant intake, and service delivery for caregiver support initiatives.Planning, Outreach & Education: Plan and implement community outreach events, training opportunities and educational programming for family caregivers and care partners.Resource Referrals: Connect individuals and families to respite care, supplemental services, and community benefits.
