Job summary

Harford County Government seeks a Community Resources Coordinator/Priority Populations Coordinator to manage caregiver support services and resource referrals for priority populations under 60 with disabilities.

Based in the Wellness Team (Office on Aging), this role coordinates intake, outreach, and reporting, ensuring accurate records and grant compliance while engaging diverse stakeholders.

Qualifications

• 2 years of experience in case management, aging services, caregiving support or service coordination for disabled populations, or related experience.
• Bachelor's Degree in Social Work, Human Services, or a related field.
• Knowledge of aging programs, disability benefits, and community resources.
• Strong communication and documentation skills.

Responsibilities

• Oversee daily operations, participant intake, and service delivery for caregiver support initiatives.
• Plan and implement community outreach events, training opportunities and educational programming for family caregivers and care partners.
• Connect individuals and families to respite care, supplemental services, and community benefits.
• Coordinate care with internal staff and fiscal teams to ensure funding availability.
• Track service utilization and prepare required reports per grant rules.
