Job summary

Lifeworks, Inc. in West Roxbury, MA is seeking a full-time Community Resource Specialist to develop and implement employment-focused curriculum and coordinate volunteer and community-based activities for individuals with ID/DD.

The role combines curriculum development, relationship-building and staff support, including safety oversight and MAP-related duties. A valid driver’s license and background checks are required, with a strong emphasis on community engagement.

Qualifications

  • Experience working with Individuals with ID/DD.
  • Bachelor's or associate degree in a related field with two years of experience, or HS diploma with five years in Human Services.
  • Must be able to pass fingerprint background check, CORI, DPPC and driving background checks.
  • Valid Driver's License required.
  • MAP certification preferred.

Responsibilities

  • Build and maintain volunteer and community-based site relationships.
  • Develop quality curriculum to be used by case managers to teach participants.
  • Perform case management duties including documentation and file maintenance.
  • Proficiency in virtual and social media platforms.
  • Research and develop community-based activities and liaise with local communities.
  • Lead on technology-based curriculum materials and activities.
  • May serve as MAP staff and safety officer as assigned.
  • Other duties and assignments to support the program.

Lifeworks is hiring a full time Community Resource Specialist to work with Individuals with ID/DD. Works with staff at LES to develop and help implement employment focused curriculum that can be presented to identify and secure a variety of volunteer, leisure and community-based activities for individuals within the program. To improve individuals' experiences by performing duties involved in planning, developing, implementing, coordinating, monitoring and evaluating community-based activities and curriculum. The role is a varied one that combines elements of curriculum development, relationship building and volunteer coordination. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.

  • Build and maintain volunteer and community-based site relationships through fostering contacts, onsite visits, as well as thorough research via telephone and web searches.
  • Develop quality curriculum that can be utilized by case managers, to effectively teach and engage participants.
  • Performs case management duties including the following: documentation of training and activities, maintenance of individual confidential files on assigned caseload, review and submit all required paperwork to supervisor according to agency procedures. Attends regular meetings as necessary. Ability to be flexible regarding necessary changes in work schedule and task as assigned by supervisor to cover the needs of the supported individual.
  • Proficiency in virtual and social media platforms
  • Research and develop meaningful and appropriate community-based activities that support the goals of the individuals' served as well as serving as a liaison with the various local business and art communities
  • Model effective teaching methods for staff to ensure active participation by the group. Provide feedback and support to staff to achieve this outcome
  • Conduct him/herself with the understanding that he/she is a representative of LES and carries the reputation of the agency with them during their interactions
  • Oversee and assist in developing future programmatic needs
  • Share information with other staff, participants, families/guardians and service providers, as required.
  • Primary lead on technology based curriculum materials and activities.
  • Assist as one of the primary MAP staff.
  • May serve as the program Safety Officer as assigned by Director and be responsible for implementing safety plans and programs as well as conducting monthly fire drills depending on the needs of the program.
  • Other duties and/or assignments to work/program locations based on the needs of the company.
Requirements:
  • Experience working with Individuals with ID/DD
  • A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Valid MAP certification is preferred
  • Excellent verbal and written communication skills
