Job summary

Kaleidoscope is a child and family services agency dedicated to helping children, youth, and families thrive. The Community Resource Manager - THRIVE will serve as a key connector between families, community partners, service providers, and the THRIVE network, focusing on building and maintaining relationships and identifying resources to promote wellbeing.

Qualifications

• Master’s degree in social work or related field required.
• MSW preferred.
• MSW: 1 year social service experience; Master’s degree in related field: 2 years of social service experience.

Responsibilities

• Coordinate THRIVE project activities, including scheduling team meetings, creating agendas, and maintaining meeting minutes.
• Build and strengthen partnerships with community organizations, service providers, and stakeholders to expand resources for families.
• Identify and address gaps in community services and improve collaboration among providers.
• Attend community coalitions, service area meetings, and partner network meetings to cultivate relationships and advocate for family needs.
• Maintain and regularly update a comprehensive resource directory organized by social determinants of health.
• Collaborate with Intact Family Services staff to identify and secure resources, including flex fund supports.
• Develop and coordinate community engagement activities and events to strengthen connections between families and services.
• Attend Child and Family Team meetings to assist with resource planning and coordination.
• Create informational materials, resource guides, flyers, handouts, and communications that promote programs and services.
• Track referrals, service connections, resource utilization, and outcomes within agency databases.
• Support continuous quality improvement through data collection, reporting, and program evaluation activities.
• Ensure compliance with THRIVE data collection requirements and reporting standards.
• Participate in supervision, team meetings, professional development activities, and department initiatives.
