Job summary

Responsibilities

• Connect members to community and care resources cost-effectively.
• Coordinate community outreach resources and promote care services.
• Support members on community and resource inquiries.
• Prepare and distribute educational materials for community care.
• Document community resources to comply with health plan policies.
• Assist clinical team with outreach, education, and member support.
• Conduct non-clinical health assessments to refer members to services.
• Assess home safety and environment to direct to services.
• Perform telephonic or in-person outreach to reach hard-to-reach individuals.
