Job summary

Jane Pauley Community Health Center seeks a Community Resource Navigator to be a primary contact for patients and communities in ambulatory care settings. You will help with benefit utilization, access to resources, and reducing barriers to services. The role will float across JPCHC clinics as needed.

You will work with diverse populations, provide guidance on Medicaid, Marketplace plans, and prescription assistance, and partner with multiple departments to improve access to care.

Qualifications

  • High School Diploma or Equivalent required.
  • Bi-Lingual/Multilingual a plus.
  • Two years of previous experience in outreach navigation preferred.
  • Customer service experience required.
  • Must be willing to travel 50% of the time to different site locations.
  • Valid Indiana Driver’s License and proof of auto insurance required.
  • Certified Indiana Navigator certification required within 90 days of employment.
  • Certified Application Counselor certification required within 90 days of employment.
  • Community Health Worker certification required within 6 months of employment.

Responsibilities

  • Work outreach events during traditional and non-traditional hours and locations.
  • Collaborate with all JPCHC departments.
  • Meet with self-pay patients to discuss payment options and complete Sliding Fee Scale.
  • Maintain confidentiality with empathy and respect.
  • Resolve patient issues by consulting with providers, plan admins, and insurers.
  • Educate members on government assistance programs and how to use the system.
  • Maintain HIPAA standards and confidentiality of PHI.
  • Follow up with clients for government assistance needs.
  • Maintain Food Pantry at designated location and manage stocking/orders.
  • Assist with Tele-Visit appointments when consumer is remote.
  • Assist patients with Medicaid and Marketplace applications and follow-up.
  • Assist patients with Medicare information.
  • Process income information for Sliding Fee Scale.
  • Assist with Prescription Assistance Programs applications.
  • Provide community resources to underserved populations.
  • Participate in outreach and health education events.
  • Support Social Determinants of Health initiatives.
  • Support special projects and other duties as required.
