Telligen is seeking a Community Liaison/Resource Navigator to connect members with community resources and non-clinical support in Illinois. The role supports care managers, builds resource partnerships, and helps members access housing and services.

Travel and driving are often required to serve Cook County and statewide needs. The candidate should have a relevant degree orCertifications, strong knowledge of social determinants of health, and a current Illinois driver’s license.

Qualificações

• High School diploma or equivalent with 3 years of experience in community-based services.
• Bachelor’s degree in social work or related field or relevant certifications.
• Certified care/case manager preferred; valid Illinois driver’s license required.

Responsabilidades

• Connect individuals with community resources and programs per service plan.
• Provide non-clinical support to care managers and monitor referrals.
• Develop and maintain a resource library for Cook County and beyond.
• Collaborate with care managers to coordinate healthcare and transition services.
• Escalate issues following established protocols.
