Job summary

Community Partners is seeking a Resource Center Specialist to connect individuals with essential services and resources across health, housing, financial assistance, and disability programs. You will guide clients to appropriate supports, document referrals, and help families navigate complex systems with empathy and clarity.

We value strong communication, reliable information sharing, and a proactive approach to problem solving.

Qualifications

High School Diploma or G.E.D. required.
A minimum of 3 years social work or case management experience in aging, disabilities, community health, nursing home or hospital discharge planning preferred.
Alliance of Information Referral Specialist (AIRS) certification. If not certified, you must become certified within one year of hire.
Excellent oral and written communication skills.
Experience in customer relations, call management, information and referral or related field.
Ability to utilize technology provided to perform requirements of the job.
Hold a valid driver’s license and proof of vehicle liability coverage.

Responsibilities

Connect individuals in need of assistance with services and resources.
Assist with linking to financial assistance, disability-related services, medical programs, rehabilitative services, vocational rehabilitation, legal assistance, housing, disaster relief, food pantries, home-delivered meals or other services/organizations.
Provide guidance and referrals to empower individuals to live independently within their communities.
