Responsibilities

• Determines resources to meet patients' and families' needs and develops a multifaceted service plan.
• Maintains a caseload with multiple patient contacts daily; serves as primary health care contact when needed.
• Advocates on patients' behalf and assists with entitlement programs and communication with external agencies.
• Conducts staff and community training on resources and keeps staff updated on procedural requirements.
• Documents obstacles to obtaining services and develops liaisons with community and governmental agencies.
