Job summary

PRIME TOP CARE CORP is seeking a trusted community member to serve as a Community Resource Liaison in the New York area. You’ll help identify families who may benefit from our Social Care Network Program, share available resources, and connect them with the care coordination team for eligibility screening and enrollment.

This is a 1099 Independent Contractor position with a flexible schedule and an hourly rate of $20–$25.

Qualifications

• Must be a trusted community member with local connections.
• Ability to identify individuals and families who may benefit from Social Care Network Program.
• Strong communication and guidance skills to connect clients with care coordination.

Job description
