  • MAP certification preferred by Massachusetts Department of Public Health.

Responsibilities

  • Support residents with daily routines, medication reminders, meals, transportation, and personal care.
  • Monitor resident behavior and document observations, progress notes, and incidents.
  • Assist with the development and implementation of individualized service plans under clinical supervision.
  • Promote a respectful, inclusive, and trauma-informed living environment.
  • Engage residents in therapeutic, recreational, and community integration activities.
  • Respond to crises using de-escalation and safety protocols; report to clinical leadership.
  • Communicate effectively with care team, family, and external providers.
  • Participate in team meetings, trainings, and quality assurance activities.
