The Pavilion at McLean Hospital, part of Mass General Brigham, is seeking a Community Residence Counselor to provide direct patient care and support within a structured therapeutic milieu. The role supports residents with daily activities, coordination of appointments, and adherence to treatment plans under supervision of nursing and clinical leadership.

Eligible candidates have at least a high school diploma (bachelor's preferred) and one year of related patient care experience; CPR training

Qualifications

  • One year of patient care or related experience, preferably in a psychiatric setting.
  • CPR/De-escalation training is required.
  • Excellent communication and teamwork skills.
  • Ability to work with a multidisciplinary team.
  • Basic computer skills; familiarity with PRISM/Epic is helpful.

Responsibilities

  • Provide direct patient care under supervision of nursing and clinical staff.
  • Coordinate patient activities and appointments.
  • Maintain clear, concise, and timely documentation and shift notes.
  • Participate in patient rounds and team meetings.
  • Support residents with daily routines, medication reminders, and transportation.
  • Follow hospital policies and ensure patient safety.

Job Summary

Under the direction of the Program Manager, the Community Residence Counselor (CRC) at The Pavilion provides direct patient care and participates in the therapeutic milieu within the scope of defined practice. Does this position require Patient Care? Yes

Essential Functions
  • Provides direct patient care in accordance with the current treatment plan under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
  • Provides and supervises community activities as needed.
  • Remains awake and alert at all times while on duty.
  • Assists patients in maintaining and keeping various appointments.
  • Completes clear, concise, and timely documentation and shift notes.
  • Establishes helpful relationships with patients and maintains professional boundaries.
  • Participates in patient rounds, status checks, and team meetings per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital and program policies, procedures, and protocols.
  • Communicates information regarding the patient milieu and therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects and physical or emotional changes to appropriate medical staff in a timely manner.
  • Encourages patients in activities of daily living.
  • Assesses sleep patterns and assists patients in developing more appropriate sleep cycles, notifying the clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to CPR and de-escalation training.
  • Attends and participates in rounds and case conferences when appropriate.
  • Maintains the physical ability required to participate in patient activities.
  • Supports residents with daily routines, medication reminders, meal preparation, transportation, and personal care as needed.
Qualifications
  • Education: High School Diploma or Equivalent required and Bachelor's Degree Related Field of Study preferred. Can this role accept experience in lieu of a degree? No.
Licenses and Credentials
  • Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred.
Experience
  • One year of patient care or related experience, preferably in a psychiatric setting 1-2 years preferred.
Knowledge, Skills and Abilities
  • Time Management.
  • Excellent verbal and written communication skills.
  • Ability to work as part of a team.
  • Maintains a commitment to professional growth and competency.
  • All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical, and administrative purposes.
  • Familiarity with Epic is preferred.
  • Ability to drive, including transporting patients, is strongly preferred and may be required based on role and location.

The Pavilion at McLean Hospital is the premiere residential program in the country providing comprehensive diagnostic evaluations for adults 18 and older with a wide range of complex psychiatric illnesses including depression, anxiety, OCD, substance use, and contributing personality issues.
