Volunteers of America Chesapeake is seeking a Community Living Counselor to support adults in rehabilitation and recovery goals within their Individualized Service Plans. You will perform community-based duties, conduct visits to homes and job sites, and coordinate referrals while maintaining thorough documentation.

You will manage a ~40-person caseload, provide crisis intervention, and collaborate with the treatment team to ensure progress and measurable objectives are met.

Formación

• High School or GED with 2 years of college or 3 years of related experience.
• Minimum 1 year of related human service activities/experience.
• Ability to work in a community-based and multicultural setting.
• Strong organizational, oral and written communication skills.

Responsabilidades

• Manage a caseload of about 40 adults.
• Assist clients in developing interpersonal and community coping skills.
• Provide crisis intervention as needed and coordinate referrals.
• Attend weekly treatment team meetings and complete progress notes.
• Travel to clients' job sites, homes, and other settings; transport as needed.
