CASES is seeking a Psychiatric Registered Nurse in the Bronx, NY. This full-time, salaried position delivers medication management and health services in the community as part of the FACT ACT team.

The role includes nursing assessments, psychoeducation, and collaboration with prescribers to ensure quality care. The ideal candidate is a NY-licensed RN with 3+ years of experience in psychiatric or related settings, highly organized, proficient with computers, and capable of coordinating care

Qualifikationen

• RN with NY license and 3+ years in adult psychiatric or related settings.
• Knowledge of community treatment, supports, and resources.
• Highly organized and self-motivated; proficient in computers.

Aufgaben

• Provides 80% of medication management and health services in the community.
• Completes nursing assessments, progress notes, and required documentation per NY OMH/DOHMH/Medicaid rules.
• Collaborates with prescribers and LPN on medication management and care plans.
• Directs LPN on medication management, education, and psychoeducation services.
• Coordinates care with families and external providers (hospitals, housing, justice contacts).
• Participates in daily and monthly nursing meetings to discuss client needs.
• Uses motivational interviewing and trauma-informed approaches in service delivery.
• Performs other duties as required by team leadership.
