Job summary

WellLife Network's ACT Team in Queens, NY is seeking a Clinical Staff Member (RN) to provide psychiatric and medical assessment, treatment, and ongoing nursing care within the ACT program. You will evaluate needs, administer medications, support treatment planning, and ensure compliance with clinical and regulatory standards.

The role involves teamwork with ACT specialists, field visits, documentation in the EHR, and rotating 24/7 on-call coverage.

Qualifications

• RN licensure in New York State
• At least 1 year of experience working with adults with mental illness
• Knowledge of ACT model and community-based care
• Computer proficiency for EMR and email

Responsibilities

• Conduct psychiatric, medical, and nursing assessments, including initial and annual evaluations.
• Administer medications and injections; monitor reactions and coordinate with prescribers.
• Develop and review treatment plans with the team and clients.
• Document progress notes in the EHR within 48 hours of contact.
• Provide crisis intervention and supportive therapy as part of the ACT team.
