Qualifications

• High School diploma or equivalent.
• Three years related work experience preferred.
• Current driver’s license, car registration and auto insurance if providing transportation to individuals receiving services.
• All state required training and certification completed in mandated timeframes.

Responsibilities

• Assist in accessing medical and health-related services for individuals receiving services.
• Coordinates and evaluates plan implementation.
• Assists in the recruitment, screening, hiring and evaluation of staff; may manage staff schedules.
• Implements and contributes to development, review and revision of programs and behavior intervention plans.
• Schedules and/or provides opportunities for quality training and activities.
• Maintains client and staff reporting records and assists with regulatory activities.
• Provides direct care in assigned program(s) weekly and travels as required.

Education

High School diploma or equivalent

Three years related work experience preferred
