Job summary

Mentor Community Services, part of the Sevita family, is seeking a dedicated Program Supervisor to oversee Direct Support Professionals in Jacksonville, FL. This full-time, non-exempt hourly role supports 4-8 individuals in residential settings or 10-15 in other programs, with a pay rate of $19.00 per hour.

The position includes supervising daily operations, supervising staff across shifts, and ensuring compliance with person-centered plans, health care needs, and regulatory requirements.

Qualifications

• High School diploma or equivalent is required.
• At least 1 year related work experience.
• Must be 18 years or older.
• Valid driver’s license and auto insurance if driving on behalf of the company.

Responsibilities

• Supervise Direct Support Professionals.
• Oversee day-to-day operation of the program, including delivery of supports and compliance with plans.
• Work 8 to 16 flexible hours per week in a supervisory capacity, with direct support hours.
• Assist with recruitment, hire DSPs in consultation with leadership, and train staff.
• Maintain records, assist with program planning, and participate in routine progress meetings.
• Ensure safety and regulatory compliance, manage funds as applicable, and support billing/Utilization processes.
