Job summary

MDH Network seeks a Program Instructor in Los Angeles to support intellectually and developmentally challenged adults at home and in the community. You’ll engage Participants in daily routines, social activities, and meaningful recreation, while following an Individual Service Plan and ensuring safety and compassionate care.

The role requires a high school diploma or GED, 1 year of relevant experience, a valid CA driver’s license, and English fluency; bilingual EN/ES is a plus.

Qualifications

• High school diploma or GED required.
• At least 1 year of experience with individuals with intellectual or developmental disabilities or related care.
• Valid California driver’s license and auto insurance; company vehicle availability if applicable (21+ for company vehicle).
• Must read, write and speak English; bilingual English/Spanish preferred.

Responsibilities

• Assist Participants in daily routines and community activities.
• Conduct initial and ongoing assessments with Participants and their circle of support.
• Document reports accurately and timely; communicate with stakeholders.
• Provide training and development support per Individual Service Plan and Health Plan.
• Promote CARE values and coordinate with team during shifts.
