Job summary

MDH Network, Inc. is seeking a compassionate Community Activities Specialist to support individuals enrolled in the In-Home Day Program in the Burbank area.

You will provide structured, individualized assistance aligned with IPP/ISP plans and focus on social skills, budgeting, self-advocacy, work habits, and community access. The role emphasizes person-centered, community-based support for individuals with intellectual or developmental disabilities, requiring a 21+ age minimum and relevant

Qualifications

• High School Diploma or GED (or equivalent).
• Must be 21 years of age or older.
• 2+ years of full-time experience supporting individuals with intellectual and/or developmental disabilities.
• 1+ year of experience supporting individuals with emotional and behavioral needs.
• Ability to read, write, and speak English; bilingual Spanish/English preferred.

Responsibilities

• Provide structured, individualized support to individuals per IPP and ISP.
• Support community integration, skill development, and independence.
• Ensure compliance with HCBS, Title 17 and Title 22 regulations.
• Support individuals in the In-Home Day Program model, returning to home as needed.
