Job summary

DELTA COMMUNITY SUPPORTS is seeking a Community Participation Supports Specialist in Levittown, PA. The role focuses on helping individuals participate more independently in their communities and increasing skills for inclusion in everyday life.

You will escort participants to activities, support goal achievement from their ISP, and assist in developing work skills for competitive employment while maintaining safety and adherence to regulations.

Qualifications

• High School Diploma or GED required; related degrees preferred.
• Experience with individuals with I/DD or behavioral health preferred.
• Must pass background checks.
• Valid driver’s license and reliable vehicle.
• CPR/First Aid / CPS training within agency timelines.

Responsibilities

• Escort and actively engage participants in community activities and events.
• Support development of ISP/NJ Plan of Care goals.
• Help participants develop work skills and explore employment.
• Maintain safe, respectful, and culturally competent interactions.
• Document progress notes and activity logs per guidelines.
• Record hours and mileage accurately.
• Ensure safety and manage incidents per procedures.
• Coordinate with families, team members, and providers.
