Job summary

Compass Palliative Care, a California-based palliative care provider, seeks outstanding California-based Nurse Practitioners and Physician Assistants with experience and/or interest in palliative care for telemedicine appointments post-hospital discharge. The program supports flexible days and hours, and providers must reside in California.

Expected hours: 30 per week. The practitioner will be assigned a patient load and will document diagnoses, manage care, and educate patients and families,

Qualifications

• CA-licensed NP or PA with eligibility to provide palliative care.
• Experience or interest in palliative care and telemedicine.
• Excellent communication and patient education skills.

Responsibilities

• Assess patients in a telemedicine environment.
• Provide palliative care and manage patient diagnoses.
• Document patient conditions and treatment plans.
• Educate patients and families and coordinate care.
• Maintain timely follow-up notes and chart entries.
