Job summary

Baystate Health in Springfield, MA is seeking an experienced Advanced Practitioner to join our inpatient palliative medicine program. You will provide patient-centered care for adults with advanced illness, working within a consultative hospital model at Baystate Medical Center.

In collaboration with Social Work, Nursing, Spiritual Care, and Physicians, you will assess and manage symptoms, discuss goals of care with patients and families, and help develop comprehensive plans of care.

Qualifications

• BC Nurse Practitioner or Physician Assistant with clinical experience in palliative care, or geriatrics settings.

Responsibilities

• Provide patient-centered care to patients with advanced illness and end-of-life concerns.
• Collaborate with an interdisciplinary team including Social Work, Nursing, Spiritual Care Services, and Physicians.
• Provide consults to patients at Baystate Medical Center in Springfield, MA.
• Assess and manage symptoms and develop plans of care; hold family meetings when appropriate.
