Job summary

MCS Puerto Rico in San Juan seeks a Community Outreach Supervisor to lead the Outreach team, ensure program activity execution, and uphold policies. You will supervise the Team Leader and Technicians, support work plans, and coordinate services for policyholders island-wide.

You will coach staff, monitor performance metrics, and ensure quality care delivery while collaborating with interdisciplinary teams to address barriers to care.

Qualifications

• Bachelor’s degree in Social Work required.
• At least four years in social work services, community outreach, or case management, including one year in supervision.
• Driver's license valid in Puerto Rico; Puerto Rico social work license/registration required.

Responsibilities

• Supervises daily activities of Outreach Team Leader and Technicians; ensures timely outreach and coordination of services.
• Reviews documentation, referrals, and case activities in electronic systems for accuracy and timeliness.
• Provides coaching, training, and performance feedback to staff to ensure quality service delivery.
• Monitors staff productivity and performance metrics; addresses gaps with management guidance.
• Oversees field activities including home visits to ensure safety and policy compliance.
• Coordinates non-clinical services by referrals to community resources and timely follow-up.
• Acts as first escalation point for complex cases and urgent situations; supports problem resolution.
• Collaborates with interdisciplinary teams to support care management and interventions.
