  • Driver's License valid in Puerto Rico; license/registration with Association of Social Work Professionals of Puerto Rico.
  • Ability to travel across the island for home visits and field assessments.

Job description

The Community Outreach Supervisor oversees the daily operations of the Community Outreach team, ensuring effective execution of program activities and compliance with established policies and procedures. This role provides direct supervision to Community Outreach Team Leader and Technicians, supports the implementation of work plans, and ensures timely coordination of services for policyholders. The Supervisor serves as a key link between staff and management, facilitating communication, addressing operational challenges, and supporting quality service delivery for high-risk and special needs populations.

ESSENTIAL FUNCTIONS:
  • Supervises daily activities of Community Outreach Team Leader and Technicians to ensure timely outreach, assessments, and service coordination. Monitors compliance with program plans and operational guidelines.
  • Reviews and ensures accuracy, completeness, and timeliness of documentation, referrals, and case activities in electronic systems.
  • Provides ongoing coaching, training, and performance feedback to staff to ensure quality and consistency in service delivery.
  • Monitors staff productivity and performance metrics, identifying trends and addressing gaps in collaboration with management.
  • Oversees field activities, including home visits and outreach efforts, to ensure compliance with safety standards and organizational expectations.
  • Supports the coordination of non-clinical services by ensuring appropriate referrals to community resources and timely follow-up.
  • Serves as the first level of escalation for complex cases, service barriers, and urgent situations, ensuring appropriate action and resolution.
  • Collaborates with interdisciplinary teams to support care management processes and effective implementation of interventions.
  • Ensure staff identify and address barriers to care and social determinant of health (e.g., transportation, housing, food, language, access to services) and facilitate appropriate solutions.
  • Maintains oversight of the community resource bank database to ensure information is accurate, current, and accessible to staff and providers.
  • Facilitates regular team meetings to review caseloads, discuss challenges, reinforce policies, procedures, and best practices.
  • Supports audits, reporting requirements, and quality improvement initiatives by ensuring compliance with program standards.
  • Supervises, monitors, trains, evaluates, and corrects personnel among others to comply with his/her role as a supervisor, to assure that his/her personnel fulfill their work plan and comply with their essential functions and that they behave according to the policies, standards, norms, and procedures of the Company.
  • Must comply fully and consistently with all company policies and procedures, with local and federal laws as well as with the regulations applicable to our Industry, to maintain appropriate business and employment practices.
  • May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document.
MINIMUM QUALIFICATIONS:

Education and Experience: Bachelor’s degree in Social Work. At least four (4) years of experience in social work services, community outreach, or case management, including one (1) year of experience in personnel supervision.

Certifications / Licenses: A Driver's License valid in the Commonwealth of Puerto Rico is required. A valid Association of Social Work Professionals of Puerto Rico License and Registration is required.

Other: This role requires occasional travel across the island, including to off-island municipalities such as Vieques and Culebra. Candidates must have a dependable vehicle in good condition and be able to conduct home visits and field assessments as part of care management support activities.
