Job summary

MCS Health in San Juan, Puerto Rico, seeks a registered nurse to perform care management, organizational determinations, and transitional care for policyholders with chronic conditions, developing comprehensive, customized care plans.

You will coordinate services, conduct health risk assessments, document progress in the electronic file, and collaborate with physicians and interdisciplinary teams to ensure timely access to care.

Qualifications

• Bachelor’s degree in Nursing Sciences.
• 2+ years of direct care of patients with chronic health conditions.
• Current Puerto Rico nursing license.

Responsibilities

• Evaluates admission criteria for Complex Care Management Programs and Initiatives.
• Evaluates policyholder’s condition to identify needs via Health Risk Assessment.
• Establishes comprehensive care plans with goals for each policyholder.
• Identifies barriers and develops short- and long-term case management strategies.
• Guides patients and families during adaptation to chronic conditions and follows up.
