Job summary

Center for Human Development, Inc. in Torrington, CT is seeking a HCWH (How Can We Help) Recovery Coach to join our community outreach team.

The role supports individuals with opioid use disorders by providing outreach, connecting to services, and promoting recovery. You will work in streets and at community sites, maintain a schedule of outreach hours, build relationships with agencies, and help participants access housing, healthcare, and supports.

Qualifications

• BA/BS preferred in human services or related field.
• HS Diploma or GED required.

Responsibilities

• Helps homeless outreach teams locate sites and provide outreach.
• Builds relationships with state, city, and community agencies/resources for opioid crisis support.
• Assists individuals with opioid use history in acquiring community services.
• Manages street outreach hours and scheduled outreach at community sites (Soup Kitchen, Gathering Place, hospital, etc.).
• Designs and participates in empowerment and recovery activities.
• Maintains records of outreach usage for individuals with opioid use disorders and their families.
