Job summary

Northwest Human Services in Salem, OR seeks a Community Outreach & Engagement Lead (CHW IV) to mentor CHWs, strengthen outreach workflows, and coordinate complex care with community partners.

You will lead weekly coverage, monitor referrals and EHR tasks, and advocate for patients while promoting health equity and access to services. This is a full-time, non-exempt, on-site role with opportunities for professional growth.

Qualifications

• OHA-approved CHW certification required.
• Three years of CHW experience and CHW IV duties.
• Knowledge of community resources and healthcare navigation.
• Ability to work independently while collaborating with multidisciplinary teams.
• Current BLS certificate for Healthcare Providers.
• Valid Oregon driver’s license and proof of automobile insurance.

Responsibilities

• Coordinate weekly team coverage schedules, floor rotations, outreach assignments, and program-specific responsibilities.
• Monitor referrals and EHR tasks to support timely follow-up.
• Promote fair and balanced task distribution across the team.
• Identify workflow gaps and communicate concerns to supervisor for improvement.
• Mentor CHWs and provide onboarding and ongoing training.
• Coach peers on documentation standards, resource navigation, and patient advocacy.
• Coordinate training aligned with patient needs and CHW scope of practice.
• Share tools and best practices to strengthen team confidence and effectiveness.
• Lead outreach events and strengthen community partnerships.
