Job summary

Catholic Community Services (CCS) invites applications for a Full-Time FHI Community Health Worker in Salem, OR. The role supports individuals and families to improve health outcomes, coordinating assessments, planning, implementation, and linkage to services.

A CHW certification is required within 12 months of hire with recertification every three years. Salary ranges from $25.08 to $31.88 hourly with a schedule of Monday–Friday, 8am–5pm.

Qualifications

• OHA CHW certification within 12 months of hire.
• Certification recertification every 3 years.
• Valid Oregon driver’s license with adequate auto insurance.
• Reliable transportation to support community outreach.

Responsibilities

• Maintain CHW certification and CHW assister certification.
• Build relationships with community members to identify priorities and mobilize action.
• Provide culturally responsive health education to support individuals and families.
• Coordinate services and referrals to address identified needs in the service plan.
• Maintain up-to-date case notes and documentation weekly.
