Job summary

Central Health in Austin seeks a skilled Community Health Worker Lead to guide CHW teams, oversee outreach activities, and mentor CHW I/II/Specialists. The role involves coordinating training, evaluating performance, and ensuring culturally competent, patient-centered care across clinics and navigation centers.

You will partner with directors and staff to expand access for high-need patients, develop workflows, and build partnerships with community organizations.

Qualifications

• Minimum Education: High School Diploma.
• 8 years experience as a Community Health Worker and ability to perform CHW Specialist duties.
• Certification as a CHW or proof of application within six months of employment.
• CHW Instructor Certification required upon hire.

Responsibilities

• Proficient in CHW Specialist duties.
• Manage creation and implementation of community outreach programs.
• Assist in hiring, onboarding, training, and coaching of CHW staff.
• Develop and monitor staff productivity; provide feedback.
• Coach CHW interns and specialists.
• Orient and assess competency of CHWs; communicate results to director.
• Determine needs for community health programs; coordinate educational calendar.
