Job summary

Access Services seeks a Community Outreach Specialist to serve Montgomery County, PA, with an office base in Plymouth Meeting. You will engage individuals experiencing homelessness and connect them with housing, healthcare, and community resources.

The role emphasizes relationship-building, field outreach, and collaboration with housing and health partners, requiring strong communication and a passion for serving underserved populations.

Qualifications

• Bachelor’s degree in public health, social work, or related field required.
• Master’s degree in public health preferred.

Responsibilities

• Conduct street-based outreach and community engagement with individuals experiencing homelessness and related challenges.
• Build trusting relationships with unsheltered individuals and assess needs to connect with services.
• Provide housing and shelter navigation to access stable housing opportunities.
• Connect individuals with healthcare, behavioral health, and other resources.
• Collaborate with healthcare providers and community partners to coordinate care.
• Provide education and referrals related to mental health and substance use disorders.
• Distribute basic needs supplies and participate in coordinated community efforts.
• Engage volunteers and community stakeholders to advance solutions.
