The role involves street-based outreach, housing navigation, collaboration with healthcare providers, and community partners to connect people with benefits and treatment options, driving to off-site locations as needed.

Qualifications

• Bachelor's degree in public health, social work, or related field.
• Master's degree preferred.
• Experience with homelessness or housing instability and substance use/mental health is desirable.

Responsibilities

• Conduct street-based outreach and community engagement with individuals experiencing homelessness.
• Build trusting relationships with unsheltered and vulnerable individuals, assessing needs and connecting to services.
• Provide housing and shelter navigation to access stable housing opportunities.
• Coordinate with healthcare, behavioral health agencies, and other partners to improve outcomes.
• Provide education on mental health, substance use, HIV, and recovery services.
• Distribute basic needs supplies and participate in community efforts to support homeless individuals.
• Engage stakeholders through outreach, presentations, and trainings to advance community-driven solutions.
• Maintain documentation and participate in meetings, staying informed about local needs.
