• Organize and participate in health fairs, community events, and outreach programs to promote health education and preventive care.
• Conduct educational sessions on chronic disease management, nutrition, mental health, and preventive care.
• Create and distribute educational materials to community members regarding healthcare resources and services.
• Advocate for patients within the healthcare system, helping them navigate services and access resources.
• Serve as a liaison between patients, healthcare providers, and community organizations to address healthcare needs.
• Maintain accurate patient records and documentation.
• Assist in scheduling appointments, follow-up visits, and referrals.
• Respect and understand the cultural diversity and specific needs of the community served.
• Provide culturally sensitive care and adapt educational materials to meet community needs.
