Job summary

The Ohio State University Wexner Medical Center in Columbus, OH is seeking a Community Care Coordinator to transition patients to home care, coordinate care, and support discharge planning within the health system.

You will review insurance and medical documentation, arrange services, and collaborate with hospital staff to ensure smooth transitions and quality patient care. Associates degree with 2+ years of experience required; RN/LPN/LSW preferred; valid driver's license.

Qualifications

  • Associates degree with minimum two years' experience required.
  • RN/LPN/LSW preferred license(s) when applicable.
  • Must have valid driver's license and auto insurance.
  • Ability to work across care settings and coordinate services.

Responsibilities

  • Identify eligible home-care patients and determine home care needs.
  • Assist discharge planning with hospital/facility staff.
  • Coordinate health care services as ordered by physicians and ensure follow-up.
  • Serve as a resource for patients and maintain facility relationships.
  • Review clinical documentation following agency and Medicare guidelines.
  • Participate in care integration meetings and collaborate with care teams.

You may attend discharge/multidisciplinary rounds in acute care, ambulatory and or other settings within the health system to share expertise and to assist the patient in transition of care from one setting to the next within the health system.**MAJOR AREAS OF RESPONSIBILITY:*** *Operations:* Identify future patients and determine home care eligibility. Review patient insurance and medical documentation. Increase awareness of services offered.* *Discharge Planning:*Assist hospital/facility personnel in the discharge planning process.* *Coordination of Care:* Coordinate health care services as ordered by the attending physician and ensure coordination of all ancillary services following patient discharge.* *Customer Service:* Function as a resource for your patients. Service account(s) to maintain facility relationships. Promote well-being of patients as a part of the facility team(s).* *Policies:*Review and complete all clinical documentation following agency protocol and Medicare/Federal guidelines.* *Collaboration:* Participate in care integration meetings. Build and maintain lasting positive relationships with patients/clients, facility/hospital personnel, physicians, and any other team members.
