Job summary

Sanctuary Personnel Limited is seeking an experienced Occupational Therapist to work in the Community Adults Team in Crewe. The role involves home visits, planning and coordinating equipment and adaptations to enable safe discharges from hospital, and supporting adults with illnesses and disabilities in the community.

You will have HCPC registration, a relevant degree, and strong communication skills. The position offers full-time ongoing work with flexible opportunities to balance professional

Qualifications

• Diploma or Degree in Occupational Therapy (HCPC registered).
• Strong written and verbal communication skills.
• Flexible, self-motivated, and able to manage workload under pressure.
• Willingness to travel to community settings and patient homes.

Responsibilities

• Assess the needs of adults with illness or disabilities within the community.
• Determine the nature and extent of required equipment and home adaptations.
• Provide informed advice and guidance to service users.
• Complete thorough assessments to support smooth hospital discharge.
• Arrange necessary equipment to support safe discharge to home environments.
• Liaise with architects regarding adaptation plans.
• Collaborate with housing, NHS, and voluntary organisations.
• Ensure adaptations meet assessed needs and refer to rehabilitation services when needed.
