Job summary
Chinese-American Planning Council (CPC) is hiring a Nutrition Navigator to support MSS in identifying and assisting eligible individuals for SNAP and related benefits. Qualifications
- Prescreen and review eligibility for SNAP and other benefits.
- Assist with SNAP application and follow-up for benefits.
- Liaise with HRA and partners to troubleshoot SNAP issues.
Responsibilities
- Review individuals’ and families’ eligibility for SNAP and other benefits.
- Assist eligible individuals with the SNAP application process and follow-up.
- Engage in mediation with HRA and partners to resolve SNAP issues.
- Process SNAP applications through AccessNYC.gov.
- Respond to information requests and follow up with callers.
- Prescreen for other entitlement benefits and referrals.
- Public speaking and outreach at community events and pop-ups.
- Act as liaison between community members and city departments.
- Prepare reports for funding sources.
- Attend meetings and trainings as directed.
